If it has been less than thirty days since your citation was issued, this form can be used to request a court hearing for an infraction case.
If it has been between 31 and 90 days since the issue date of this citation and you still wish to request trial, you can print a PDF copy of the form and submit the form either by mail or in person along with a $16.00 late fee.
If it has been over 90 days since the issue date of the citation, you may only request trial if you file a Defendant Motion along with the Request For Trial Form and submit it either by mail or in person.

This form is used to file a change of address whenever the defendant's current address is different from the address on the citation.

This form is used to request action by a judge or hearing officer. The most common requests are motion to set aside driver's license suspension and set for trial and a motion to request continuance for a court hearing.

This form is used to elect a traffic school. Election of a traffic school course will result in withholding of points against your driver's license. You may request a four hour traffic school once per year and only five times in a lifetime. If you are not eligible you may request an eight hour traffic school course once in a lifetime. In order to make a traffic school election, please submit this form either by mail or in person along with the traffic school civil penalty payment. The traffic school will collect the registration fees.

The Guilty Plea and Waiver of Court Hearing Form allows the defendant to plead guilty to the charge without showing proof of compliance, namely driver's license and/or vehicle registration. This option may be used if the defendant is charged with Failure to Exhibit Driver's License on Demand, Florida Statute, 322.15 and Driver's License Expired 4 Months or Less, Florida Statute 322.065. The defendant may also use this option if charged with Expired Registration, Florida Statute 322.07 (3)(a) and/or No Certificate or Registration Florida Statute 320.0605. There is a civil penalty of $108.00 for this option and a guilty disposition will be reflected on the defendant's driving record.

This form is used when a defendant suspects that someone has been fraudulently using his/her driver's license. This form must be filled out, notarized, and submitted either by mail or in person with a motion to the judge requesting an identification hearing. The defendant may include any supporting documentation for consideration with the motion.

This form is used when a defendant wants to change his/her previous election within 30 days. Defendants may change from guilty upon payment of a civil penalty, to a traffic school election. In order to change your election, fill out the form and submit it either by mail or in person with any additional payments necessary.

This form is used when the defendant claims they were not driving the vehicle when it went through a toll without paying. At the time of the violation the tag may have been stolen, or that they are not the owner of the vehicle. The defendant must include the information of the person in control of the vehicle at the time the citation was issued so that the citation may be transferred. The form must then be notarized and submitted either by mail or in person. This form can be filed only for SUNPASS Violations.

This form is used when an individual is accepting responsibility for a citation when the citation was originally cited to the owner of the vehicle. The individual must fill out the form completely and submit it either by mail or in person along with the civil penalty of $53.00 or request a court hearing. This form can be filed only for SUNPASS Violations.

This form is used so that defendants can get a copy of their driver's record from the Division of Highway Safety & Motor Vehicles. This form must be filled out and submitted in person and will include a fee of $6.10 for the first page and $1.00 for each additional page.

This form is used for defendants who cannot pay the full balance of monies owed on or before the due date. The form must be completed and received with 25% of the balance due on or before the due date and must be submitted either by mail or in person. This will only extend the due date for an additional 30 days.

This form is used for defendants who desire a court hearing but cannot personally appear in court. The form must be completed in full and a bond in the amount of the civil penalty must be submitted in the form of a cashier's check or money order. This form must then be notarized and submitted either by mail or in person. Upon completion of the case, the court will notify you of the outcome. If fines and or court costs were assessed, they will be deducted from the bond. If the case is dismissed or you are found not guilty, your bond amount will be refunded.

E-Filing is being rolled out as a pilot and initially only the attorney's selected for the pilot are allowed to use the e-filing functionality. E-filing is expected to be opened to all attorneys, related agencies, and pro-se defendants within 90-120 days.
