However, if you are unable to locate the records online, you may submit a Public Records Request form. This form may be submitted by mail to Records Management, Miami-Dade County Clerk of Courts, P.O. Box 14695, Miami, FL 33101 or by email to firstname.lastname@example.org.

- Each request received will be assigned a control number
- Upon receipt of the request, the requestor will receive an acknowledgement of receipt
- All subsequent related correspondence should be submitted to email@example.com
- The Public Records Request form is exclusively for submitting public records requests only. This form is not to be used for other inquiries or questions.
